
Medication Management
Sunshower provides telehealth psychiatric evaluation and medication management for eligible patients located in New York, Iowa, and Minnesota. Every service begins with clinical assessment and shared decision-making. Not every option is appropriate for every person, and recommendations depend on your history, symptoms, goals, medical considerations, and state-specific requirements.
Medication visits focus on what has changed, what is helping, what is getting in the way, and whether the current plan still makes sense. We review symptom relief, functioning, side effects, adherence, sleep, relevant medical issues, and any new risks or concerns.
The aim is to find the simplest effective plan—not to add medication automatically. Recommendations may include starting, adjusting, continuing, tapering, or reconsidering medication, along with coordination with other clinicians when appropriate.
Prescribing
When medication may help, treatment begins with careful selection based on your symptoms, diagnosis, medical history, previous treatment, preferences, and potential interactions. We discuss expected benefits, common side effects, alternatives, and how your response will be monitored.
Prescription Monitoring
Medication response, side effects, mood, sleep, appetite, functioning, vital signs, laboratory results, and safety are monitored over time. Controlled-substance treatment may also include prescription-monitoring database review, treatment agreements, toxicology screening, or coordination with other prescribers when clinically appropriate.
Comprehensive Medication Reconciliation
We conduct a detailed review of all prescription medications, over-the-counter products, vitamins, supplements, allergies, and previous adverse reactions—including treatments from other prescribers. This process identifies discrepancies, duplications, interactions, dosing concerns, and opportunities to simplify and improve the safety of your medication regimen.
Deprescribing
Deprescribing is a careful, collaborative process for reducing or stopping medications that are no longer helping, causing side effects, or contributing to polypharmacy. When appropriate, we may use slow, individualized hyperbolic tapering—progressively smaller dose reductions as the dose decreases—to minimize withdrawal effects and distinguish withdrawal from symptom recurrence.
Care coordination
With your authorization, Sunshower can collaborate with your therapist, primary-care clinician, specialists, pharmacy, and other members of your care team. Coordination helps identify medication interactions, reduce duplicated treatment, and keep psychiatric and medical care aligned.
Medication Assisted Treatment
Medication support may be available for alcohol or opioid use disorders, including naltrexone when clinically appropriate. Treatment may include psychiatric assessment, harm-reduction education, relapse-prevention planning, safety monitoring, and coordination with therapy or a higher level of substance-use treatment when needed.
